Application for Supplementary Exam Permit

Application for supplementary exam permits is a service for students who wish to apply for approval of the application for permission to submit supplementary exams. Permission to take follow-up exams that are permitted and recognized by the Management Study Program - Undergraduate Program is divided into 4 categories with the file requirements, namely:

  • Inpatient Hospital
    • – Stamped Parental Statement 
    • – Stamped Student Statement 
    • – Hospitalization Certificate 
    • – Proof of medical record (lab results)
    • - Recipe
    • – Proof of Hospital Payment
    • - Course schedule
    • – Photocopy of KTM
  • Hajj/Umrah
    • – Stamped Parental Statement
    • – Photocopy of Visa
    • – Photocopy of Ticket
    • – Travel Agent
    • - Course schedule
    • – Photocopy of KTM
  • Faculty/University Envoy
    • – Stamped Student Statement
    • – Faculty/University Representative Letter
    • - Course schedule
    • – Photocopy of KTM
  • Death of the Nuclear Family
    • – Death Certificate
    • – Nuclear Family Certificate
    • - Course schedule
    • – Photocopy of KTM
    • – Photocopy of Family Card
